NECK PAIN: THE TRUE CAUSE

As a chiropractor, I often see patients who come in complaining of chronic or recurring neck pain. Understandably, most assume the issue lies within the neck itself — stiff joints, tight muscles, or arthritis of the spine. But here's something many people don’t realize: a large portion of neck pain may actually originate from a lack of mobility in the thoracic spine — the mid-back as well as the rib cage.

How the Mid-Back Affects the Neck

The thoracic spine and rib cage provide the structural foundation for the neck and shoulders. When it becomes stiff or immobile — often due to prolonged sitting, poor posture, poor breathing patterns or simply lack of movement — the neck is forced to compensate. This compensation can result in excess strain on the cervical spine, leading to tension, headaches, and even nerve irritation.

When your thoracic spine moves well, it can help relieve pressure off of your neck and allow for healthier movement patterns overall. This is one of the key reasons we assess total body function as a whole rather than treating the neck in isolation.

Signs Your Thoracic Spine Needs Attention:

  • Difficulty rotating your torso
  • Rounded shoulders and forward head posture
  • Recurrent neck or upper trapezius tension
  • Limited overhead shoulder mobility

2 Simple Exercises to Improve Thoracic Mobility

Here are a two of my favorite exercises you can do daily to help restore and maintain mobility in your mid-back:

1. Thoracic Extension Over a Foam Roller
Targets: Mid-back extension

Tip: Avoid extending from your lower back — keep the movement focused on the thoracic spine.

2. Open Books
Targets: Rotational mobility

Final Thoughts

If you’ve been chasing neck relief with little success, it might be time to look lower — at your thoracic spine. By improving mobility in the mid-back, you can reduce compensatory strain on the neck and promote healthier, pain-free movement throughout the upper body.
